Confidentiality
At Hands on Hartford, we value privacy and confidentiality. While serving as a volunteer, we expect that you will
value and respect the privacy and confidentiality of program participants and staff, and agree not to discuss or
disclose any aspects of their personal, social, medical or financial information with anyone other than the project
leader and Hands On Hartford staff.
Agency Policies
Hands on Hartford does not tolerate any form of sexual harassment, or use of alcohol and/or illicit drugs on volunteer
work sites. Hands On Hartford does not discriminate on the basis of race, color, religious creed, age, sex, marital status,
national origin, ancestry, mental retardation, past or present history of mental disorder, learning disability, physical

Release from Legal Claims
In connection with my voluntary involvement in activities undertaken for, and with the participation and support of Hands
On Hartford, a nonprofit organization, I hereby agree, for myself, my heirs, assigns, executors and administrators to release
and discharge Hands On Hartford, its officers and directors, employees, agents and volunteers (the “Hands On Hartford Parties”)
from all claims, demands, and actions for injuries sustained to my person and/or property as a result of my involvement in such
activities, even where caused by the negligence of the Hands On Hartford Parties, and I agree to release and hold the Hands On
Hartford Parties harmless from any cause of action, claim or suit arising there from. I hereby attest that my attendance and
involvement in such activities is voluntary, that I am participating at my own and sole risk, and that I have read, understand,
and accept the terms and conditions of this release.
Permission to Use Images and/or Recordings
I hereby give Hands On Hartford Parties my consent to use any photograph, videotape, digital image, and/or sound recording made
of me or in which I may be included in whole or part. I grant permission to the Hands On Hartford Parties to use such images
and/or recordings for any purpose and in their discretion. Additionally, by signing this document, I release and forever discharge
the Hands On Hartford Parties from any and all claims or liabilities of any kind which may arise from the use of my likeness.
I acknowledge and agree that I will not receive any compensation in connection with any use of my likeness.
Criminal Charges
I hereby confirm, represent and warrant that I have never been convicted of or charged with any crime involving or relating to
child abuse or neglect, child pornography, child abduction, or any other violent offense, including kidnapping, rape or any sexual
offense, nor have I ever been ordered by a court to receive psychiatric or psychological treatment in connection with such crime or crimes.
Involvement with programs that serve children and other vulnerable populations may necessitate a criminal background check prior to
volunteering. If so, a notice to this effect will be included in the project description.
Volunteer Agreement
Hands on Hartford values the time and effort of volunteers. I agree to honor my volunteer commitment on the agreed upon scheduled date(s)
and time(s). If I am unable to fulfill my commitment for any reason, I will give ample notice to the project leader.
Submission of this form indicates that I understand and will abide by the policies listed above, including the need to protect the privacy
and confidentiality of program participants.
